At St. Joseph Medical Center, we are committed to addressing our communities’ needs and have aligned our processes to follow CMS Health Equity guidelines to ensure patients are screened and getting the resources they need to continue their journey of recovery once they leave our facility. St. Joseph Medical Center actively provides information on community resource centers and health centers to help address Social Determinants of Health (SDOH) needs and provide health education to the community. St Joseph Medical Center is committed to collaborating with community partners to help improve the overall health of our community and strives to provide culturally and linguistically appropriate health care services that meet the needs of those we serve.
