At St. Joseph Medical Center, we are committed to addressing our communities’ needs and have aligned our processes to follow CMS Health Equity guidelines to ensure patients are screened and getting the resources they need to continue their journey of recovery once they leave our facility. St. Joseph Medical Center actively provides information on community resource centers and health centers to help address Social Determinants of Health (SDOH) needs and provide health education to the community. St Joseph Medical Center is committed to collaborating with community partners to help improve the overall health of our community and strives to provide culturally and linguistically appropriate health care services that meet the needs of those we serve.
Commitment to Health Equity
St. Joseph Medical Center (SJMC) is committed to offering our diverse patient population the best care possible regardless of race, ethnicity, language (spoken/written), sexual orientation, gender identity, or disability status. To ensure we provide the best care possible and a fair opportunity at optimal health outcomes, background information is gathered from patients, as well as their language preferences/needs. All information shared is confidential.
SJMC utilizes this information to analyze and stratify data to help identify health disparities and implement improvement strategies with the goal of decreasing disparities among the identified patient populations. SJMC previously focused its work on two metrics, one related to the reduction of readmissions and the other addressing maternal care. Currently, SJMC is focusing on maternal care for its health equity initiatives and meets regularly with the Health Equity Committee, Medical Executive Council, and Board to report on current progress.
